During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 12/16/2024 and completed on 12/16/2024. The on-site inspection began at 8:30 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were 41children present and 12 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Child 1?s immunization documentation was dated 2 days after Child 1?s first day of attendance.
Plan of Correction: Staff will obtain before a child attends the center.
Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

Staff 1 TB screening was dated three months prior to date of employment; Staff 1 has been employed for over a month.
Plan of Correction: Staff will obtain within the required timeline.
Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening.

Staff 3 and Staff 5 updated screenings were due 3 months ago. The updated screenings have not yet been completed.
Plan of Correction: Staff will obtain updated screening.

When food is brought from home the food container must be clearly labeled in a way that identifies the owner.

A blue lunchbox and a pink lunchbox for the Nido Room were not labeled in a way that identified the owner.
Plan of Correction: Lunchboxes were labeled during the inspection.
Standard 8VAC20-780-90-A
A written agreement between the parent and the enter shall be in each child?s record by the first day of the child?s attendance. The agreement shall be signed by the parent and include: An authorization for emergency medical care; A statement that the center will notify the parent when the child becomes ill, and the parent will arrange to have the child picked up as soon as possible; and a statement that the parent will inform the center within 24 hours after the child or household member as developed any reportable communicable disease.

The records for Child 1, Child 2, Child 3, Child 4, and Child 5 did not contain the required written agreements.
