Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 09/30/2024 and completed on 10/02/2024. The on-site inspection began at 9:30 am and ended at 10:30 am. The inspector reviewed compliance in the areas listed above. There were 24 children present and 2 staff. The inspector reviewed 5 children?s records onsite, and 4 staff records electronically on 10/02/2024. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

1The record for Child #1, enrolled for one month, was missing the following required components: work phone number of each parent; address of two designated people to call in an emergency if the parent cannot be contacted.
