Inspector Notes
An unannounced complaint inspection was conducted on 2/8/2024 from 10:35am to 12:50pm in response to a complaint that was received by the department regarding background checks, emergency preparedness, and subsidy vendor agreements. Staff interviews, documentation review, and observations were made. The preponderance of evidence does support a valid finding regarding standards related to background checks, emergency preparation, and subsidy vendor agreements. Areas of non-compliance are identified on the Violation Notice.

Violations
4Evidence: There were no central registry findings and criminal history records from every state staff # 5 and staff #7 resided in the preceding five years.
Based on review of employee files, the center did not obtain central registry findings within 30 days of employment.
Evidence: Central registry findings for Staff #2, 3, 5, 6, 7 and 8 were obtained more than 30 days after employment.
Based on review of evacuation records, the center did not implement a monthly practice evacuation drill.
Evidence: There was no record of a practice evacuation drill for December 2023.
Based on discussion, and confirmed with the center, the vendor was not ensuring compliance with the terms of the vender agreement.
Evidence: The center was in possession of 3 VA-ECC cards and had asked cardholder for the PIN. 9.7 of the vendor agreement states: "Only parents, guardians and alternate cardholders named by the parent(s) of children receiving child care assistance are authorized to possess and swipe the VA-ECC card issued to them on the Vendor's POS machine or use the IVR system to report attendance. The Vendor shall not be in possession of or swipe any parents' or guardians' VA-ECC card or use the IVR system to report attendance; nor shall the Vendor ask the cardholder for his Personal Identification Number (PIN)."
