During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated on 1/24/2025 and completed on 1/24/2025. The on-site inspection began at 10:35am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. There were 67 children present and 16 staff. The inspector reviewed six children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Staff #3, who had been employed for one month, did not have a completed national criminal background check.
Plan of Correction: The center will obtain results of a criminal history record check for the staff.
Standard 22.1-289.057-A
The program is required to submit to VDH and implement a plan to test potable water for lead.

The program did not have evidence of a testing plan submitted to VDH.
Plan of Correction: The center will submit a plan of action to test their water for lead.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The record for Child #4, enrolled for one month, was missing the following required components: work phone number of each parent who has custody.
Plan of Correction: The center will obtain the work phone number for the parents.
