There were 19 children present with one staff.
The children were doing homework and eating a snack.
Two child records and two staff records were reviewed.

Based on a review of staff files, the facility failed to ensure that all repeat fingerprint background checks are conducted every five years. Evidence: 1. There is no repeat fingerprint background check on file for Staff #1. The last fingerprint on file is dated 10-4-18
