Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis screening
at the time of employment and prior to coming into contact with children. The documentation shall be completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department. The record of staff #4 contained documentation of a TB screening that was dated two months and 10 days prior to employment.
Plan of Correction: Per the Director: Will have the employee do a new screening.
Standard 8VAC20-780-240-A
The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment. The record of staff #1 contained documentation of the Virginia Department of Education- sponsored orientation dated 100 days after the date of employment.
Plan of Correction: The training class has been completed.
Standard 8VAC20-780-510-L
Medication, except for those prescriptions designated otherwise by written physician's order, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children. Multiple medications were observed unlocked in a kitchen cabinet.
Plan of Correction: Per the Director: We will add a lock to the cabinet door.
Standard 8VAC20-780-530-A-1
At least one staff in each classroom or area where children are present shall have current certification in cardiopulmonary resuscitation (CPR) as appropriate to the age of the children in care from an organization such as the American Red Cross, American Heart Association, American Safety and Health Institute, or National Safety Council. The training shall include an in-person competency demonstration. 1. Staff #5 observed alone and in charge of a group of children had CPR certification that expired 25 days ago. 2. Staff #6 observed alone and in charge of a group of children had CPR certification that expired two months ago. There was no documentation of current CPR certification for staff #5 and staff #6.
Plan of Correction: Per the Director: Staff are scheduled for a CPR class.

At least one staff in each classroom or area where children are present shall have current certification in first aid from an organization such as the American Red Cross, American Heart Association, American Safety and Health Institute, or National Safety Council. 1. Staff #5 observed alone and in charge of a group of children had first aid certification that expired 25 days ago. 2. Staff #6 observed alone and in charge of a group of children had first aid certification that expired two months ago. There was no documentation of current first aid certification for staff #5 and staff #6.
