Inspector Notes

An announced initial inspection was conducted on 8/17/2022 for the Y After School at Grandin Court Elementary. Staff records, emergency plans, policies & procedures and staff orientation & training were all reviewed. Physical facilities were inspected including outside play area, bathrooms and general care areas.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

An exit interview was conducted with the Program Director on 8/17/2022.

Evidence: An emergency radio was not accessible on the day of the inspection.
