An unannounced inspection was initiated on 5/6/2025 and completed on 5/14/2025 in response to a self-reported incident received by the licensing office on 5/1/2025 related to behavioral guidance and forbidden actions. This inspection included interviews, document review and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Behavioral guidance shall be constructive in nature and age and stage appropriate. Staff #1 did not use appropriate behavior guidance to redirect Child #1 (age 6). Staff #1 pushed the chest of the child twice to move the child away from them.
