Inspector Notes

An unannounced code compliance inspection for the religiously exempt child day center was conducted on 11/28/2023 a complaint inspection was also conducted on 11/28/2023. A total of 47 children, ages 2 years-5 years and 8 staff were present during the inspection. A total of 5 children?s records and 5 staff records, as well as other required documents were reviewed during the inspection. The children were observed during physical activities, mealtime, and self-directed activities. One inspector and one administrator arrived at approximately 11:30 am and departed at approximately 1:15 pm.

Information gathered during the inspection determined non-compliance with the Code of Virginia and violations were documented on the violation notice issued to the provider.

Based on record review and staff interviews, the center failed to ensure that upon enrollment of a child in a regulated child day program, the center shall require that the person enrolling the child presents the program with information regarding previous child day care and school attended by the child and proof of the child?s identity and age.

Evidence: Staff 4 verified there was no information obtained from person enrolling the child regarding previous child day care and school attended by the child or identity and age for Child 1, Child 2, Child 3, Child 4, and Child 5.
Plan of Correction: Provider will obtain information from families upon enrollment.
