Inspector Notes

An unannounced 60 day monitoring inspection was conducted today from 11:20am-1:00pm. There were 31 children (infants-5yrs.) directly supervised by 9 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, playing outside, napping, and eating lunch. Diapering and hand-washing procedures were also observed. There was an adequate supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Based on record review, the tuberculosis (TB) screening/test documentation that the center obtained for a staff member was not completed within the last 30 calendar days of the date of employment.

Evidence:
Staff #3's record contains a TB screening/test that was completed on 11/26/2021. The staff member's first day of employment was documented as 11/28/2022.
Plan of Correction: Staff member has submitted updated TB test.
Standard 8VAC20-780-270-A
Based on observation, areas of the center were not maintained in a safe and operable condition.

Evidence:
1.The front wall and the side wall of the shed located on the outdoor playground were not connected. The gap between the two walls was approximately 1 ft. wide and approximately four ft. long.
2.The sink located in the school-age bathroom has a broken faucet.
Plan of Correction: Work orders have been submitted.

Based on observation, prepared infant formula was observed without a date.

Evidence:
In the infant classroom, two bottles of prepared infant formula were observed in the refrigerator labeled with the child's name, but without a date.
Plan of Correction: Bottle was labeled during visit. Teachers have been retrained and reminded to check bottles upon drop off.
