Inspector Notes

A monitoring inspection was initiated on 12/14/2023 at approximately 8:35am and concluded on 12/14/2023 at approximately 10:20am. There were a total of 36 children with 11 staff members present. Children were observed during story time, painting, readying, and engaged in group activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 6 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of staff information, a central registry search was not completed for staff supervising children.

Evidence: There was no record of a central registry search for Staff #2.
Plan of Correction: The center will obtain a central registry search for the staff member.

Based on inspection of the building, areas of the center were not maintained in a safe condition.

Evidence: Curtain chains in classrooms were a head entrapment for the children in care.
Plan of Correction: The center will secure the curtain chains so they no longer pose a head entrapment risk.
Standard 8VAC20-780-60-A
Based on review of children's records, work phone numbers of each parent who has custody were not kept for each child enrolled.

Evidence: Work phone numbers for both parents who have custody of Child #1 were not kept on record.
Plan of Correction: The center will obtain work phone numbers for both parents of the child.
