Inspector Notes
An unannounced monitoring inspection was conducted 2/5/24 in response to a self report incident where a child was served a food containing a known allergen and was subsequently treated for an allergic reaction.
Observation of the classroom and staff interviews were conducted, one staff file and 1 child's file were reviewed. Area of noncompliance are identified in this report.

3Evidence:
The facility failed to ensure that the menu for 1/29-2/2/2024 was provided to parents by drop off 1/29/2024, which is used for parents to identify foods that may contain allergens
Evidence:
On 1/29/24 Child#1 was fed (ravioli) containing eggs, which is a documented allergen for the child.
Based on record review, the center did not maintain and keep at the center a record that contained for an enrolled child, the name, address, and phone number of a second designated person to call in an emergency if a parent cannot be reached.
Evidence: Child#1?s record did not have a second emergency contact person listed.
