Inspector Notes

An unannounced monitoring inspection was conducted 2/5/24 in response to a self report incident where a child was served a food containing a known allergen and was subsequently treated for an allergic reaction.

Observation of the classroom and staff interviews were conducted, one staff file and 1 child's file were reviewed. Area of noncompliance are identified in this report.

Based on record review and staff interviews, the center failed to post or give parents the menu listing foods to be served for meals and snacks during the week of 1/29-2/2/2024.
Evidence:
The facility failed to ensure that the menu for 1/29-2/2/2024 was provided to parents by drop off 1/29/2024, which is used for parents to identify foods that may contain allergens

Based on record review and staff interviews, a child with a diagnosed food allergy was served food identified in the written care plan as an allergen.
Evidence:
On 1/29/24 Child#1 was fed (ravioli) containing eggs, which is a documented allergen for the child.
