Inspector Notes

An unannounced investigation was conducted on 4/12/24, following a self-report to the Fairfax Licensing office. The incident occurred on 4/10/24 and involved a child at the center unsupervised. Observations were made, documentation was reviewed, and interviews were completed with staff. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the center did not have documentation of a negative tuberculosis screening for all staff.
Evidence: Staff #2's Date of Hire (11/6/2023) record did not have documentation of a negative tuberculosis screening.
Plan of Correction: Center provided staff TB screening dated 11/1/2023.

Based on observation, electrical outlets did not have protective covers in areas used by children.
Evidence: The Multipurpose room, used by children under the age of 5 years old had 2 uncovered and un used outlets.
Plan of Correction: Outlets were covered during inspection.
Standard 8VAC20-780-340-F
Based on a self report, the staff did not ensure that children under 10 years of age are always within actual sight and sound supervision of staff.
Evidence: During group time in the multipurpose room. There were two staff supervising sixteen children between the ages of 2-3 years old. Child#1 (3 years old) ran out of the room and was approximately 25 feet, behind a closed door, out of sight and supervision of the two staff reportedly for two minutes on 4/10/2024.
Plan of Correction: The center will ensure exit to multipurpose room is supervised/accessed by staff.
