Inspector Notes
An unannounced renewal inspection was conducted from 10:45 am pm to 11:55 am. At the time of entrance, 5 children totaling 10 points were in care with the provider. The record sample size consisted of 5 children's records and the provider's record. The children in care were observed engaging with the provider, participating in educational activities and outdoor play. There have been no injuries since the last inspection and no medications are being administered at this time. Areas of noncompliance are noted in the violation narrative and were discussed with the provider during the exit meeting at the conclusion of the inspection.
Violations
2Evidence:
1. There was a weather band radio in the provider's emergency bag however; the radio did not work.
2. The provider reported that they were unsure as to how to operate the radio during this inspection.
Evidence:
1. The record for child #3(enrolled 3/1/23 and present during inspection) did not contain documentation that the child has been adequately immunized.
2. The documentation available did not contain a doctor's signature or statement that the child had been adequately immunized.
3. Provider confirmed that this documentation was not available during the inspection.
