Inspector Notes

An unannounced renewal inspection was conducted on 7/11/2023 from 8:30am to 10:20am. There were 10 (2-5yrs) directly supervised by 2 staff. The physical plant, 2 staff records, 5 children?s records, no children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group and individual activities. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Based on observation, the facility did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence:
1) observed that there were two access boxes with water valves accessible to children in the main preschool room, and the small "green" room. The main preschool box had tape keeping it shut, and the green room box would not close.
2) observed that there were 2 nap mats with tears in them. One nap mat had tears along the seam, and the other nap mat had tears along the body of the mat, exposing the absorbent material beneath.
Plan of Correction: 1- So we notice after the inspection there were locks attached on the water supply access boxes. (I sent a video and pictures for the inspector) but we didn't know there was a lock so they were not locked. we had tape for the one was located i the preschool room. The green room was still close we were not using it yet their was no kids in it. Both boxes got locked right away and was sent to the inspector.

2- The teacher didn't pay attention about the mat we notified the parent at pick up and they replaced the mat with a new mat.

Parents: The center was clean and nice from the inside its just the code terms that falls under gets label like this !!!
