Inspector Notes

A complaint investigation was initiated on November 4, 2022 at 11:00 a.m. to 2:00 p.m. in response to a complaint received by the licensing office regarding injury reports.

Staff interviews were conducted. Classroom observation was conducted. Documentation was reviewed.

Based upon the information gathered during the inspection, noncompliance with applicable standards or code sections was determined and a violation was documented on the violation notice issued to the program.

Based on staff interview and review of documentation, it was determined that the center failed to notify parents of any known minor injuries by the end of the day. Evidence: On October 12, 2022 Child A, age 2 years old, got her finger stuck in a toy tambourine. As a result, Child A sustained a small cut on her finger that bled. Staff did not notify the parents of the minor injury. The parents of Child A contacted management regarding the injury.
