Inspector Notes

An unannounced non-mandated inspection, as required by the Conditional License, was conducted on 10/19/2022 for the Y After School at Highland Park Elementary. There were 23 children, ages 5 -12 years and 2 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 5 children?s records and 2 staff records were reviewed. The children were observed during arrival, snack time and during a guest speaker. The inspector arrived for the inspection at 2:15pm and departed the center at 3:19pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

The inspection findings have been updated as a result of a review completed by the Licensing Administrator on 3/1/2023

Based on review of two staff?s records, the center failed to ensure that an employee was not employed until the center had the person?s out of state criminal history and central registry results.

Evidence: Staff #1 was hired on 6/20/2022 and on the day of the inspection did not have the required out of state criminal history check and central registry results completed.
Plan of Correction: Provider will obtain out of state criminal history and central registry checks for staff.

Based on observation and discussion with staff, the center failed to implement a monthly practice evacuation drill.

Evidence: Documentation of monthly evacuation drills was unavailable and on the day of the inspection there have been no emergency evacuation practice drills completed during the first two months of the school year.
Plan of Correction: Provider will complete and document practice evacuation drills monthly.
