Inspector Notes
A subsidy health and safety inspection conducted on 2/15/2023. There were 15 children, ages 5 -11 years and 3 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 4 children?s records and 2 staff records were reviewed. The children were observed during active time on the playground. The inspector arrived for the inspection at 3:30pm and departed the center at 4:32pm.
A licensing inspection was also conducted on this date. Both inspections combined cover health and safety regulations. Only subsidy requirements not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

1Evidence: Staff #2 date of hire was 02/25/2022 and on the day of the inspection did not have the required Preservice Training.
