Inspector Notes

An unannounced monitoring inspection was conducted on 9/18/2023 for the Y After School at Highland Elementary. There were 14 children, ages 5 -10 years and 3 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 2 children?s records and 2 staff records were reviewed. The children were observed during craft time and pick up. The inspector arrived for the inspection at 4:20pm and departed the center at 5:00pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on review of two staff?s records, the center failed to ensure that an employee was not employed until the center had the person?s out of state criminal history results prior to the 1st day of employment.

Evidence: Staff #2 was hired on 1/20/2023 and on the day of the inspection did not have the required out of state criminal history results documented.
Plan of Correction: Provider will submit for out of state criminal history results.

Based on observation and discussion with staff, the center failed to implement a monthly practice evacuation drill.

Evidence: The most recent emergency evacuation drill was conducted on 5/31/2023 and on the day of the inspection no emergency evacuation drills were conducted during the first month of school.
Plan of Correction: Provider will conduct monthly emergency evacuation drills.
