Inspector Notes
An unannounced renewal inspection was conducted today from 2:10pm-2:30pm. There were 104 children directly supervised by 14 staff. The physical plant, 7 staff records, 10 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed eating lunch and transitioning to nap. There was an assortment of age appropriate materials for the children in care. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

2Evidence:
The chair rail in the Early Preschool room was observed to have chipping paint and splintering wood in the area next to the cot storage nook.
Evidence:
1. Child 1's record did not contain the place of employment for one of the listed parents.
2. Child 6's record did not contain the place of employment for two of the listed parents.
We have since already gone through all files to make sure they are complete with place of employment for parents. [We will] make sure files are complete and watch over to prevent from occurring again.
