Inspector Notes

An unannounced complaint investigation was conducted today from 10:35am to 11:15 am in response to a complaint received by the Fairfax Licensing Office about injury documentation, nutrition, and daily activities for children.

Interviews were conducted, documentation was reviewed, and an observation of the two year old classroom was conducted.

A preponderance of the evidence supported the complaint related to injury documentation and the complaint is deemed to be "Valid". VIolations can be found on the violation notice.

Based on interviews and documentation review, the center did not ensure a written record of children's serious and minor injuries.

Evidence:
There was no documentation for Child #1's repeated bite marks. Staff confirmed that child regularly was seen with bite marks and no reports were present on the day of inspection about the bite marks.
Plan of Correction: To ensure proper training for teachers children on documentation of bite marks or other injuries seen or unseen. Paper training incident reports.
