Inspector Notes

An unannounced monitoring inspection was completed on02/12/2024 as a part of the licensure period. The on-site inspection began at 1:00pm and ended at 2:30pm. The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. There were 5 children present and1 caregivers; the point total was 11. The inspector reviewed 2 children?s records and 1 caregiver/household member(s) record.Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

The provider shall maintain an up-to-date record at the family day home for each enrolled child to include the name, address and telephone number of two designated persons to
contact in case of an emergency if the parent cannot be reached.

Child 2, who was in care at the time of inspection did not have documentation of 2 emergency contact addresses.
Plan of Correction: The provider will obtain this information today at pick-up and document it in the file.
