Inspector Notes
An unannounced inspection was conducted 04/23/2024 in response to a complaint regarding staff-to-children ratio requirements. Documentation was reviewed, classroom observations and interviews were conducted. The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Based on observation, interview, and documentation, the center did not ensure compliance with ratios of staff to children as required.
Evidence:
Based on review of the ?Child Supervision Record (CSR) dated 04/19/2024 documented at 8:30 a.m. there were 15 children in the Infant II room with 3 staff (S1, S2, and S3)
Based on observation the center did not maintain the ratio of 1 staff to four children in the Infant II room on 04/23/2024. The licensing inspector entered the Infant II room at approximately 8:19 a.m. and observed 13 infants being cared for by 3 staff members. The staff acknowledged the room was out of ratio when licensing inspector entered the room as well as other times on other days. A fourth staff member entered the classroom at approximately 8:23 a.m.
