The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. S5, employed for more than 4 months at the center, did not have documentation of requesting a central registry check from one state.
Plan of Correction: S5 transferred from another center within the company. Upon review, we confirmed that she has not lived in Maryland within the last six years. Because she did not reside in that state during the required five-year lookback period, a Maryland central registry check was not necessary. Her background check file has been updated to reflect this clarification, and all documentation is now complete. Date corrected 11/07/25

Plan of Correction: C2 is no longer enrolled at the center. The family of C9 has been contacted, and updated immunization records will be submitted no later than 12/12/2025. Moving forward, weekly file audits will be conducted to ensure all required immunizations are collected and maintained on time.

Plan of Correction: Families have been notified that physical exams are required for continued enrollment. Updated physicals will be received by 12/12/2025. Enrollment auditing procedures are being tightened to prevent repeat violations.

Plan of Correction: We have adjusted morning staffing to ensure ratios are met during drop-off transitions. A designated opener has been assigned to the gym with backup support scheduled from 7:00 AM-7:30 AM. Staff have been retrained on ratio requirements and documentation.

The center shall implement a monthly practice evacuation drill. A drill was not documented in October 2025.
Plan of Correction: The October drill was completed but not logged; documentation has now been entered. Going forward, the director or designee will document fire drills the same day they occur and upload them to the drill folder for cross-verification.
