Inspector Notes
An unannounced inspection was initiated on 08/12/2025, 08/19/2025, and completed on 08/26/2025 with a local agency in response to a complaint received by the licensing office on 08/01/2025 relating to allegations of administration and programs. This inspection included documentation review and interviews with center staff.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid.
Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the
violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/05/2025. A POC submitted after this date will not appear on the public website.
Violations
2The licensee shall ensure that the center?s activities are maintained in compliance with it?s own policies and procedures. The center documented that S4 ??physically turn the child around at the handwashing station in a manner that appeared forceful?. The form further documented that S4 ??was re-educated on the center?s polices and expectations regarding physical contact with children. He was reminded that staff are to use verbal communication and positive guidance strategies when interacting with children and that physical redirection is not permitted.?
Repeat Violation
The following actions or threats thereof are forbidden: physical punishment and rough handling. Staff stated in interviews conducted 08/19/2025 and 08/26/2025 that S1 was observed punishing and rough handling C1 and C2 on multiple occasions by ?popping? them on different parts of their bodies, ?flicking them?, ?grabbing them?, and ?pulling them? and causing them to cry.
