An unannounced inspection was initiated on 7/25/2023 in response to a self report that was received by the licensing department on 5/4/2023 regarding allegations of allegiantly forbidden actions. On 7/25/2023, the inspector was on site from approximately 9:45 AM until 11:00 AM. Interviews with staff were completed, and observations were made. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. There were no violations issued.
