Inspector Notes

A subsidy health and safety monitoring inspection was initiated on January 24, 2024 in response to a complaint that was received on January 5, 2024 in reference to child allergies. Documentation was gathered and interviews were conducted. Only subsidy requirements that are not covered in the licensing standards were reviewed during this supplemental inspection.

Violations were cited as a result of this inspection.

Based on record review, and interview, the vendor failed to obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
The record for child #1, date of enrollment 8/21/23, revealed a food allergy. The record did not contain a written care plan for child #1 to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Interviews with the vendor confirmed that the record did not contain a written care plan for child #1.
Plan of Correction: I will make sure each child with a food allergy listed on their registration form has a written care plan from their physician before starting.
