Inspector Notes
An unannounced renewal inspection was conducted on March 5, 2024 from approximately 10:30 am to 12:00 pm. The inspector reviewed compliance in the areas of administration, personnel, household members, physical environment and equipment, emergencies, caregiver training, preventing the spread of disease and nutrition. The provider record, 1 caregiver record, 1 household member record, and 10 children?s records were reviewed. All areas of the home were observed and areas where the children are cared for were inspected. There were 11 children in attendance for a total of 22 points with 2 caregivers supervising The children were observed learning about magnets, reading stories, reviewing the days of the week and colors, and playing outside while the inspector was on the premises.
The first aid kit was inspected and found complete. The menu, emergency numbers, and daily schedule were posted.
The information gathered during the inspection determined violations with applicable standards or law.
Violations were issued.

Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).
Violations
3Evidence: The record for caregiver #2, date of employment 02/12/24 indicated that caregiver #2 has lived in another state outside the state of Virginia in the past 5 years. The record for caregiver #2 did not contain a sex offender registry check and the results of a criminal history record information check from that state.
Evidence: The record for caregiver #1, date of employment 02/12/24, contained a tuberculosis screening dated 09/22/23.
Evidence: The records for child #1, child #2, child #3, child #4, and child #6, did not contain the names of persons other than the custodial parents who are authorized to pick up the child.
