Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/24/2024 and completed on 9/25/2024. The on-site inspection began at approximately 4:11 pm and ended at 5:11 pm. The inspector reviewed compliance in the areas listed above. There were 24 children present and 3 staff. The inspector reviewed 5 children?s records on site and 4 staff records electronically on 9/25/2024. This inspection included document review, a tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/2/2204. A POC submitted after this date will not appear on the public website.

Staff records shall be kept for each staff person and contain the name, address, and telephone number of a person to be notified in an emergency which shall be kept at the center and information to be kept at the center, about any health problems that may interfere with fulfilling the job responsibilities.

Staff #1, who had been employed for approximately four months, did not have emergency contact information and health information at the center.
