Inspector Notes

A monitoring inspection was conducted on 11/29/2022 with the center director and staff. There were 15 children present, ranging in ages from 5 years to 12 years with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of the attendance record, the facility failed to maintain a written record of daily attendance that documents the arrival and departure of each child in care. Evidence: on 11/29/2022, the attendance record had 12 children marked as present, however, there were 15 children in care.
Plan of Correction: The attendance record was corrected. It will be checked for accuracy each afternoon.
