Inspector Notes

A monitoring inspection was conducted on 12/04/2023 with the center director and staff. There were 22 children present, ranging in ages from 5 years to 12 years, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed. The children were working on homework with tutors and playing a group game in the gym. Staff files were reviewed electronically 12/5/2023.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of three staff records, the facility failed to have each staff complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment. Evidence: Staff 1 (date of employment 2/21/2023) did not complete the VA Pre-service orientation course until 6/1/2023.

Based on review of four staff records, the facility failed to have required documentation in each file. Two references as to character, reputation, and competence should be checked prior to employment. Relatives by blood or marriage and people who are not knowledgeable of the individual, such as recent acquaintances, shall not be considered objective
references. Evidence: Staff 1 (date of employment 2/21/2023) had two references that are relatives. Staff 3 (date of employment 6/20/2022) had two references that were not identified as to relation.
