Inspector Notes

This monitoring inspection was initiated by licensing staff and concluded on 10/19/2023, between the hours of 10:30 am to 12 pm. There were 4 children present (12 points), ranging from infant to preschool-age, with 1 caregiver supervising. The children were engaged in the following: learning, story time, music, nap time, bathroom breaks, hand washing, toy play, getting ready for lunch, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 5 children records, 1 staff record, and 4 household records were reviewed.

Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Based on review, 3 of 5 children files indicated that each child emergency contact information was not reviewed on an annual basis.

Evidence -
1. On the date of inspection (10/19/2023), the last documented review of the emergency information for the following children were older than one year: Child #1 (dated - 09/19/2022), Child #2 (dated - 09/25/2022), and Child #3 (dated - 10/12/2022).
2. This finding was confirmed on the date of inspection by the Provider.

Standard 8VAC20-800-90-C
Based on review, 1 of 5 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years.

Evidence -
1. On the date of inspection (10/19/2023), the current immunization record on file for Child #4 was dated - 09/28/2022.
2. Based on information on file, Child #4 was under the age of 2 years on the date of inspection.
