A monitoring inspection was conducted on 02/17/2023. A tour of the facility was conducted, in addition to a record review and interviews with staff. The inspector emailed the director/provider a list of items required to complete the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the

Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe and operable condition.

The handle to one of the drawers in the kitchen was broken.

Based on observation the center did not ensure all Hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

There was an unlocked bottle of air deodorizer on top of the towel dispenser in the bathroom.
