Inspector Notes

An announced Initial Inspection was conducted from 10:45 to 12 :00. The Inspector reviewed background checks, sample forms, permits, emergency plans, and blank children's registration forms. Room measurements were also taken and observation of supplies were reviewed. The Program has access to two more bathrooms and water temperature was also checked. Standard 8VAC20-780-330-B
Based on observation, the Playground does not have sufficient fall zone under and around equipment or materials used.
Evidence:
1. The monkey bars do not have sufficient resilient surfacing under the bars.
2. The logs situated around make shift sandbox are close together and close to the cement walkway. The logs closeness to each other does not encompass sufficient area to protect a child in the event of a fall off of the log and onto another wooden log.
Plan of Correction: We will continually rake the mulch around fall zones as necessary. We have decided to block off the use of two pieces of equipment. The logs, which are around the sandbox, will be covered over the sandbox and logs to avoid use. We will add what items are prohibited for use on the playground in our playground safety plan.

Based on the observation of the sandbox play area that does not have drainage possibility, it was determined that the area is not covered when not in use.
Evidence:
1. Sandbox area was open and no cover observed and there was a lot of debris on top and in the sand.
Plan of Correction: Children will not be permitted to use the sand area and we will add that in our playground safety plan.
