Inspector Notes
An unannounced monitoring inspection was conducted on 2/7/2024 from 11am to 12:40pm. There were 11 (Toddlers to 4yrs) directly supervised by 3 staff. The physical plant,4 staff records, 4 children?s records, no children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, playing outside, and listening to stories. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. 4Evidence: observed hand sanitizer accessible to children on a shelf in the 3-4 year old room.
Evidence: reviewed the facility's records and found that they did not have an injury prevention plan or an annual update.
Evidence: observed a sunscreen bottle in the 3-4 year old room did not have a name on it.
Evidence: reviewed the facility's emergency evacuation drill log and found that it was missing documentation of weather and notification method.
