Inspector Notes

An unannounced monitoring inspection was conducted 4/30/2024 from approximately 11:35am-12:33pm. There were 14 children directly supervised by 3 staff in two classrooms, divided by age and in compliance with ratio. The physical plant, 1 staff record, 2 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and eating snack. There was an assortment of age appropriate materials for the children in care. Areas of non-compliance are identified in the Violation Notice.

Based on record review and interview, The center did not have two or more references as to character and reputation as well as competency were checked before employment.
Evidence: Staff record had two references that were completed after date of hire. (DOH: 3/17/24, DOR: 3/31/24 and 4/5/24)
Plan of Correction: The Director will ensure at least two references are completed prior to hire date.
