Inspector Notes
An unannounced on-site non mandated monitoring inspection was conducted on 01/09/2023. There were 20 children, ages 7 months-5 years, and 9 staff members present during the inspection. The inspector observed the following: lunch, diapering, and preparing for nap. A total of 3 children?s records and 2 staff records, were reviewed. The inspector discussed the following with staff: attendance requirements, medication and using the model from for enrollment.

Evidence:
There were three bottles two sitting on the counter and one in the refrigerator that were
not dated with today?s date.

1. There was a baby food jar of Harvest Turkey Dinner in the refrigerator with no name and no
date. The jar was previously opened and partly consumed.
