Inspector Notes
An unannounced inspection was initiated on 12/18/2025 and completed on 1/12/2026, with a local agency in response to a self report received by the licensing office on December 18, 2025, relating to allegations of forbidden action. The inspector reviewed 1 child?s records and 5 staff records on-site and electronically. This inspection included document review, tour of the facility, and interviews. The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
