Inspector Notes

A monitoring inspection was initiated on 4/4/2024 and concluded on 4/4/2024. There were 12 children present with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child's records and 5 staff records were reviewed. The inspection started at 09:30 am and concluded at 12:15 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

REPEAT VIOLATION

Based on record reviews, the center failed to ensure that staff have a central registry finding within 30 days of employment.

Evidence:
Staff #3 (hire date 11/29/2023), Staff #4 (hire date 11/29/2023) and Staff #5 (hire date 8/31/2023) do not have a central registry checks on file.
Plan of Correction: Director will have central registry check completed on staff.
Standard 8VAC20-780-245-L
Based on record reviews and interviews, the center failed to ensure there shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.

Evidence:
Staff #1, Staff #2, Staff #3, Staff #4 and Staff #5 did not have documentation of completion of a daily health observation training. Interview of staff confirmed the training had not been completed.
Plan of Correction: Staff will be scheduled for training.
Standard 8VAC20-780-270-A
Based on observation, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
There is a small area of flooring in the preschool/school-age room that is missing, revealing concrete below.
Plan of Correction: Director will contact building owner to have floor repaired.

Based on review of administration records, the center failed to ensure injury prevention procedures shall be updated at least annually based on documentation of injuries and a review of the activities and services.

Evidence:
No documentation of a review of the activities and services were found in administration records. Staff was unable to provide documentation.
Plan of Correction: Director will implement plan to review activities and services to prevent injury.
