Inspector Notes

An unannounced code compliance inspection was conducted on (11/15/2023). There were 46 preschool and toddler children present, ranging in ages from 15months to 5 years, with 10 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision and procedures. A total of 3 children and 6 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

The facility is reminded that a complete renewal application must be submitted prior to expiration of the exemption on December 11, 2023. Renewal applications should be submitted through the online portal available on the public website at childcareva.com.

Based on observation and staff interview, the religious exemption program has not posted the fact that it is exempt from licensure in a visible location on the premises.
Evidence: The religious exemption letter dated 12/14/2022 was not posted in a visible location on the premises.
Plan of Correction: The religious exempt letter will be posted.

Based on observations and record review, the center is not in compliance with applicable laws and regulations with regard to building codes.
Evidence: Upon the inspector's arrival at 9:30 am there were 46 children in attendance, including 1 Primary class and a young and older toddler classrooms.
The religious exemption letter dated December 14, 2022 permits the program to provide services to 36 children.
Plan of Correction: Management will contact the Building Official to get an updated Building Inspection Request form to allow more children. The form will be emailed to the inspector.

Based on a review of staff background checks, 1 staff's search of central registry has not been completed by the end of the 30th day of employment.
Evidence: Staff #2, date of employment 6/2023 does not have documentation of the results from central registry search.
Plan of Correction: Management will contact central registry search to locate the results for staff # 2.
