Inspector Notes
An unannounced monitoring inspection was conducted today.
There were 11 children present (25 points) with 2 caregivers.
Children were observed washing hands eating lunch, and laying with a wide variety of toys and games.
Provider's certification in first aid and CPR expired on 9/18/23.
3 child and 2 caregiver records were reviewed.
See Violation Notice for non-compliances cited today.
First aid supplies, emergency supplies, menu and bathroom supplies were all in compliance today.

Caregiver 1's record contain an expired First aid and CPR certification. Certification expired 9/18/23.
Based on a review of records child record were incomplete.
Evidence:
3 child records were reviewed.
1. 3 of 3 reviewed were missing documentation of viewing proof of birth. Signature of person viewing the document, date document was viewed. (Children 1, 2, 3)
2. Signed Emergency care authorization. (Child 2)
Based on a review of records, child records did not include acknowledgement of all policies and procedures.
Evidence:
3 child records were reviewed.
1. 2 of 3 records did not include a completed decision to administer medication policy. While the form is signed, there was no policy was identified.
