Inspector Notes
An unannounced renewal inspection was conducted on 05/08/2023, in addition to an inspection in response to a self - reported incident. The inspection was conducted between 8:45AM and 11:30AM. 63 children were directly supervised by 15 staff. The physical space, 3 staff records, 3 student records, injury reports, attendance and required posted documents were inspected. Children were also observed in morning activities, interacting with each other and staff members. Age appropriate materials were available throughout all classrooms. Based on observation, hazardous substances such as cleaning materials, insecticides, and pesticides were not kept in a locked place using a safe locking method that prevents access by children. Evidence: A cabinet under the sink, in a classroom, was unlocked with keys hanging in the lock. 5 bottles of cleaner were in the unlocked cabinet.

Based on interviews, during supervision, staff did not always ensure the care, protection and guidance of the children. Evidence: 1) Child A was seen in the hallway, outside of a classroom door, without properly being transitioned into that class. 2) Child A was without supervision for approximately 30 seconds
