Inspector Notes
A monitoring inspection was conducted on 04/09/2024. 57 children were directly supervised by 12 staff. The physical space, 4 children's records, 4 staff records, attendance, evacuation drills and required posted documents were reviewed. Medications were in compliance. Children were observed washing hands, eating snacks, playing outside, engaging in age-appropriate materials and interacting with the staff. Interactions were positive. Developmentally appropriate materials were available in all classrooms.

Standard 8VAC20-770-60-C-2
Based on record review, the licensed child day program did not have a central registry finding for a staff member within 30 days of employment. Evidence: 1 out of 4 staff records reviewed did not contain central registry findings. Documentation for Staff A was sent in within 30 days of employment, but the finding was never returned and follow up was not completed.
Plan of Correction: We will follow up to obtain the central registry findings for Staff A and we will send them to the licensing inspector once obtained.

Based on record review, the center did not obtain documentation that a child had received immunizations before the first day the child attended the center. Evidence: 1 out o 4 child records reviewed did not contain immunizations prior to the first day of attendance. Child A's folder was missing immunizations.
Plan of Correction: We will obtain a copy of the immunizations right away.
