Inspector Notes
A monitoring inspection was initiated on February 15, 2023 and concluded on February 16, 2023. There were 16 children present, ranging in ages from 20 months to age 6, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
4Evidence: The records for Child #3 and Child #4, first date of attendance for both was 1-2-2023 and there was not documentation of a physical.
Evidence: The following records have documentation of TB screenings conducted outside of the required timeframe.
1)Staff #1?s date of hire was 1-2-2023 and a TB screenings/ test conducted was not conducted.
2) Staff #2?s date of hire was 1-2-2023 and a TB screenings/ test conducted was not conducted.
3) Staff #3?s date of hire was 1-2-23 and their TB screenings/ test conducted was dated 10-16-2020.
Evidence: In the Toddler Room, Child #1 has a diagnosed food allergy and staff were aware of the allergy however, there was not an allergy list available for staff to review in a confidential location.
Evidence: An epinephrine auto-injectors, was observed and it did not have the dosage amount and the time or times to be given. Center staff confirmed the epinephrine auto-injector belonged to Child #1 and it had not been administered.
