Inspector Notes
An unannounced monitoring inspection was conducted from 8:20 AM -10:30 AM. At the time of entrance there were 14 children were in care with 3 staff members present. The sample size consisted of 5 children?s records and 3 staff records. Children were observed during morning drop off, preparing for their morning routine, playing outside and preparing for their snack.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

6Evidence: The following records have documentation of TB screenings conducted outside of the required timeframe.
1) Staff #1?s date of hire was 5-15-2023 and a TB screenings/ test conducted was dated 12-8-2022.
2) Staff #3?s date of hire was 6-14-2023 and a TB screenings/ test conducted was dated 6-29-2022.

Evidence: The record for Staff #2, (date of hire 3-15-23) did not include documentation of completing the required orientation.
Evidence: The record for Staff #3 (date of hire 6-15-23) did not include documentation of completing the required orientation to include all required topics.
Evidence: The center owner confirmed the center?s policy for medication did not include the following;
1. Include any general restrictions of the center.
2. For nonprescription medication, be consistent with the manufacturer's instructions for age, duration, and dosage.
3. Include duration of the parent's authorization for medication, provided that it shall expire or be renewed after 10 work days. Long-term prescription drug use and over-the- counter medication may be allowed with written authorization from the child's physician and parent.
4. Include methods to prevent use of outdated medication.
Evidence: The following records were missing information as listed below;
1.The record for child #1 was missing for both parents listed their home phone number and their employment location; both emergency contacts listed are missing their addresses.
2.The record for child #2 was missing both parent?s employment location, the address for the 1st emergency contact listed and a 2nd emergency contact name, address and phone number.
3. The record for child #4 was missing one parent?s employment location and the addresses for both emergency contacts listed.
Evidence: The following records were missing information as listed below.
1) Staff #1, (date of hire 5-15-23) designated as a program lead for the toddler room, is missing documentation in their record to demonstrate they have the training/education required for program lead, an emergency contact name, address and phone number and documentation of any health-related problems that may interfere with the job.
2) Staff #3, (date of hire 6-14-23) is missing documentation of two references conducted prior to hire.
