Inspector Notes
An unannounced monitoring inspection was started in the center on February 21, 2024 from 9:00 AM until10:05:00 AM and concluded on February 21, 024. At the time of entrance, 19 children were in care with 6 staff members. The sample size consisted of 5 children's records and 5 staff records. Children were observed playing outside on the playground and working on puzzles and eating snack.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation and staff interviews, the licensee did not ensure staff records were available for review and completed with all required documentation.

Based on record review and staff interviews, the licensee did not ensure that each staff member shall submit documentation of a negative tuberculosis screening prior to employment and coming in contact with children.
Evidence: The records for Staff #1, #2, #4, #5 and #6 did not include documentation of a negative TB screening and were observed working during the inspection. Staff #2 began working on 2-20-2024 and Staff #1, #4, #5 and #6 began working on 2-21-2024.
Evidence: Staff #1 confirmed the most recent copy of the health inspection available for review was dated 11-14-2022.
Evidence: Staff #1 am Staff #2 were observed on the playground with 12 preschool age children and Staff #1 and Staff #2 do not have documentation of meeting program leader qualifications.
Evidence: Staff #1 and Staff #6 were observed as the only persons supervising 12 preschool children on the playground and they do not have a current certification in CPR as required

Evidence: Center Director verified they did not have documentation of a lock down drill practiced in 2023.
Evidence: There was not an attendance record for the 7 children observed in the two?s room or an attendance record for the 12 preschool children observed on the playground.
