During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on and co09/12/2025 completed on 09/12/2025. The on-site inspection began at 1:20pm and ended at 3:10 pm. The inspector reviewed compliance in the areas listed above. There were 16 children present and 5 staff. The inspector reviewed 5 children's records and 5 staff records onsite. This inspection included document review, tour of the facility,interviews,observationsand measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation
cited on the violation notice. Specify how the violation will be or has been corrected. Submit your
POC within five business days from today, which will be the close of business on 09/25/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-160-C
At least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members and individuals from independent contractors shall obtain and submit the results of a follow-up tuberculosis screening as stated in subsection B of this section. Staff #3 TB tuberculous test was more than 2 years old. TB test should been completed one month and three weeks ago.

Repeat Violation
There shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children. There was no staff present with Daily Health Observation/ Health Screening course.
Plan of Correction: We will take this Daily Health screening during our Teacher Workday.
Standard 8VAC20-780-530-A
At least one staff in each classroom or area where children are present have CPR and First aid. Staff #2 was alone in the classroom with children and did not have CPR and First Aid.
Plan of Correction: Corrected first part. Second part of CPR will be done 9/27/2025.
Standard 8VAC20-780-90-A
A written agreement between the parent and the center shall be in each child's record by the first day of the child's attendance.The agreement shall be signed by the parent. Child #1 and Child #3 did not have written agreement signed by parent.
Plan of Correction: The consent forms were given to parents and have both been signed.
