An unannounced inspection was initiated on 09/05/2025and completed on 09/25/2025, in response to a complaint received by the licensing office on 08/26/2025 relating to allegations of administration , physical plant, and background checks. The inspector reviewed 5 staff records on-site. This inspection included document review, tour of the facility, interview, and observations.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
