Inspector Notes

An unannounced, on-site monitoring inspection was initiated and concluded on May 16, 2024. The on-site inspection began at 9:15am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were 13 children present and 5 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and a violation was documented on the violation notice issued to the program.

Standard 8VAC20-780-210-A
Staff program leaders shall be at least 18 years of age, have fulfilled a high school program completion or the equivalent, and have six months of supervised programmatic experience. Two program leaders did not meet qualifications and were working at the center during the time of the inspection.
1. The records provided for Staff #2 and Staff #3 identified as a program leaders, did not include documentation to demonstrate that each individual possesses the education, certification and/or experience required by the position of program leader.
2. Staff #1, Director, confirmed that both Staff #2 and Staff #3 did not meet the qualifications for program leader.
Plan of Correction: The center responded with the following: The director will ensure that she and qualified staff will lead the classrooms. Classes will be combined if needed. Director will make sure that lead teachers are qualified.
