Inspector Notes

An unannounced inspection was initiated and completed on 6/23/25 with a local agency, in response to a complaint received by the licensing office on 6/17/25 relating to allegations of supervision, nutrition and special care and provisions. The inspector reviewed 3 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility,
interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Though not a part of the original complaint, a violation that was not reported was found related to records, and is documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/1/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.
1. On 3/31/25, a child in the infant classroom was fed another child's milk bottle by a staff member.
2. Staff # 1 stated that on 3/31/25 Staff # 2 did not follow the center's policy, and did not check the name on a milk bottle that was given to Child # 1. The milk bottle belonged to another child.
3. Staff is required to check the name on each bottle and say it out loud before feeding it to any infant. Staff # 2 did not follow the procedures and fed Child # 1 someone else's milk.
Plan of Correction: This is a very serious matter, and to ensure full compliance with licensing regulations, all staff have been retrained to follow our center?s policies. We have also implemented additional supervision measures to ensure continued adherence.
Standard 8VAC20-780-60-A
The center must verify identity for each child in care within 7 days of enrollment.
The center did not obtain any documentation of proof of identity for Child # 2 who has been enrolled at the center for three weeks.
Plan of Correction: We will remind parents to bring us proof of birth at enrollment.
